Hi, first post, I'm a new member but I've been reading here for a while. I have no where else to look for a solution, in my opinion there isn't one, but heres the prob.
I got 3D Vision months ago, and it worked, some problems here and there ghosting flickering and such. I didn't mind that though it still worked. Then sometimes the screen would white out at 120hz and I'd get frustrated roll back the drivers and it would work. Well now, for weeks now maybe months I have not had 3D Vison cuz my monitor wont display anything above 60hz. When I put it up the screen freezes and slowly whites out and the screen looks all cross hatched like. Like you can see the pixels or something. BUT this just happened, JUST happened one day without any catalyst which is what pisses me off the most.
Anyone else have this? What's the problems? Is there a fix? And please don't tell me samsung f!@ked me over. =[
I can provide video on it on my youtue channel if it helps anyone.
and if this post is in the worng place then sorry
One thing I do find weird though is that when I do this and the screen freezes and whites out, it obviously gives you the option to confirm your decision to accept the changes on the new refresh rate. If I take a screen shot when it whites out the press ESC to exit and return to 60hz then paste the SCR into paint it looks normal, well the way it should look.

Well, if you can't get your LCD above 60hz, what does that tell you?
It suggests 2 things to me.
1.) It seems your display may defective. The only way to be truly sure is to hook it up to a different rig and see if you can achieve a res higher than 60hz.
In any case, how long have you owned it? Do you know what the warranty period is?
Ok, that said, let's not get to hasty with it being defective just yet, it is possible, but not likely it's your display, so lets move on.
2.) You mention you tried rolling back drivers, which ones? Read this thread and see if you get anything out of it.
Try an older driver which you can get @ Guru3d.com, they have every Nvidia/ATI driver for all the major OS'es from the very first to the most recent.

Is you monitor being detected correctly by the drivers? It actually sounds like there may be some type of EDID detection problem..

I get pixely screens often. I have taken the factory overclock off my bfg oc edition. Reducing the card from 750 down to 680 works well for me, eliminates most problems, plus the card still never goes above 60% gpu usage. I once had many problems with a cable to the monitor which cause problems such as you explained. Couple things to try
