Setup is 2 480GTXs with 3 acer GD235HZs (DVI) plus my tv as a 4th monitor(DVI - HDMI).
The issue is that when trying to Span displays with Surround, the nvidia control panel chooses the adapter with the tv on it as the primary. I'm forced to pull the cable out of the tv to go into Surround mode with my 3 monitors.
I believe the first time I got things to work, I was able to do so by moving cables around behind my computer but this is not something I'm able to do easily and moving between Surround Mode (that I use for gaming) and Activate all monitors mode (that I use for watching movies on my tv) can put me right back into the same situation.

have you tried simply changing the order of the cables? i.e. plug the tv into the last slot instead of the first slot?

I can simply unplug my tv from the computer every time I want to go into Surround mode. That just seems silly, though. Why do the drivers want to force me into using a different 3 monitors each time I try to use Surround? If they'd simply let me choose which is monitor 1, 2, 3, and 4 I'd be set.
I think my solution is going to be to sell off the monitors, 3d vision kit, and a videocard and just go back to a single 480 and a pair of 30" monitors. I like the idea of surround gaming but unless I keep this box dedicated to that and only that, it doesn't seem to be worth the hassles.
