GOG unfortunately updated my version to 1.12.1 and after reinstalling the fix, it seems some decals broke. I can post a screenshot if needed. But right now I'm standing at the "Destroyed Bastion" west of the army camp, east of the "road to bald mountain" wp. It looks like the position of the vine has very large offset between the two eyes.

Are you sure? I launch from Galaxy all the time and usually don't have problems with the fix. The only time I need to reapply the fix is when Galaxy updates the game, or when I update the driver.
If you have the GOG version DO NOT start the game from GOG Galaxy browser! It will automatically overwrite the d3d11.dll and dxdgi.dll with it's own version and thus will make the fix invalid;)
If this happens just apply the 3D Vision fix again;)

I have been playing it for the past couple of weeks with Drivers 358.50 and it has worked perfectly. The only issue I have is that I have to import the custom profile before each launch. Has anyone else experienced this problem and solved it? It would be nice to be able to save my convergence settings.

Have the same problem... The damn Driver seems to fuck things up for some reason... It likes to re-write the profile... I was unable to see why exactly...
I will post my profile (where you can add your custom convergence in a bit). ;) So, far the only reliable way for me is to import the profile everytime I start the game.
