I have similar issues to DHR,
specifically:
1.) First, I dumped the profiles using that NV(3D) Profile Manager and added in the profile you (Helifax) posted. I'm still using the 364.72 driver set and there is/was no default profile for the game, nor did I have the EXE attached to any other profile. I re-imported the profiles successfully and double checked with NV Inspector to make sure it was available and set exclusively to the proper EXE.
2.) Then, I dumped the files from 3DMigoto in the BIN64 folder.
3.) Next, I fired up the game and it wouldn't even launch! Tried it multiple times. I went back to
the games folder and removed 3Dmigoto and tried again, the game started right up, 3D kicked in and thought I was home free.
Once the game loaded to the point where I could take control, there was no depth at all, despite the fact that 3D kicked in on it's own it was simply a mono image.
4.) So, I put 3DMigoto back in, but this time I used NV Inspector to remove the EXE from the profile (made a copy of the exe and assigned it to the profile as a place-holder) which, of course, made the game 'profile-less' (or 'profile challenged' for all you 'PC' nutz, and no - not the 'personal computer' affiliation.)
5.) Tried the game again, it loaded and 3DV kicked in but this time I noticed that one of the eye's was completely washed out, like just white, not black like DHR's issue but the result is the same.
Since I am playing the game in surround, I used the Cheat Engine method to fix the menu and that part worked without a hitch.
6.) Back to NV Inspector, I reassigned the EXE to the profile and again, one eye not working. I disabled 3DV Auto and switched to 3D-Compat Mode and that worked ok.
7.) Not satisfied however, I tried the profile I use for Batman Arkham Knight which is the "Assassin's Creed IV (SP)" and while 3DV Auto rendered either in mono or one eye blank (can't remember which) 3D Compat mode kicked in fine and looks great, not 3DV Auto great, but not bad either.
So, there you have it, If anyone is looking for a "quick fix" and either want to try or are ok/satisfied with using 3DV Compat mode, simply assign the games EXE to the 'AC IV (SP)' profile and you are good to go.
I'm hoping we can find a fix for the 'one eye blank' issue, this game is really pretty! But, I am grateful to be able to play it even in 'fake' 3D.

5760 x 1080
5860 x 1080 (Bezel Corrected)
I settled on 5760 x 1080 because that allowed me to keep the settings at:
Temporal AA = on
V-Sync = On
Textures = Very High
Shadows = High
Details = Very High
Motion Blur = Off
Forgot what else but those are the main settings anyway, but will edit the rest in later, if necessary.

I have one black eye during loading time (so long!!!) and when in game, I can't access menu in 3D, I have to switch back to 2D to see the menu.
