Any Ideas as to why my frame rates would drop from 40-80 in major cities and 100+ anywhere else to 17-20 in capitals and 27-30 frames outside?
The only change I have made is the addition of 3d vision.
The important bits are below, full dxdiag system report is attached.
My ram is 3x4GB Triple channel 1600
Processor is OC'ed to 3.5GHz
and I am using the dx11 flag for wow as well
on a different but related note, dx11 water and reflection effects seem to be rendering horribly in 3d, is this a known issue?

What settings are you running at?
Turn on triple buffering, because 3D vision halfs your framerate and forces VSync, so 80fps becomes 30fps. Tripple buffering makes it nearer 40.
As a side note WOW seems to do a lot better performance wise in 3DVision with the DX11 renderer, but it has a few additional issues if you use it, notably nameplates are at screen depth and you lose the 3D mouse cursor.
In case your wondering if going SLI will improve performance, it doesn't, you need a faster single card for WOW, it has appauling SLI scaling.

I already have triple buffering and vsync enabled, i have all settings set at the maximum setting, I have read in other forums that unchecking reduce interface lag will also bolster performance

You definitely don't want reduce interface lag enabled, it forces a GPU/CPU sync at the end of every rendered frame which is just throwing performance away.

i disabled it on my laptop, and wow! i went from 8-9fps in org to 40-50fps, its a bit spiky but i'm running it on ultra (minus shadows and sunshafts) instead of good, hopefully the performance increase on my pc at home will be as drastic!

Partol, I just use the game itself for framerate information, in wow pressing ctrl+r will display it, one of the addons I use, titan panel also displays framerate

Do you have ambient occlusion on? It dips hard when you have that on.
Also "Antialiasing - Transparency" has a huge impact as well when you have supersampling on.

In Afterburner, my max frame rate is 60fps, on-screen, in all games, when 3D is enabled.
When 3D is turned off, my max frame rate changes to 120fps.

If you are playing with 3D vision off, then
in the nvidia control panel,
unselect "Enable stereoscopic 3D" and performance should improve a little.
