Good news! DarkStarSword's latest (1.4) fix works again with NVidia's 378.49 driver. :)
Are you sure your not using CM mode the new 378.49 have a CM profile for Watch Dogs 2 and when using DSS 1.4 patch it defaults to the CM Watch Dogs 2 profile.
I tried the DSS 1.4 patch and it is broken with the latest drivers.

I can't play in CM mode (it gives me headache) so, yes I'm sure I'm using the real 3D.
After installing the latest NVidia driver, the game defaulted indeed to CM, but I immediately switched to proper 3D. (the green text mentioning DSS's fix was an extra proof).
Fact is you still need to keep the shadows to medium. First time I started the game, after installing the latest NVidia driver, I've increased them to ultra and it seemed the fix was still working okay.
But, after your post, I started the game again - with the shadows on ultra - and noticed that, when panning the camera the fix is indeed unstable.
So, again, the fix works BUT with the shadows set to medium.
Also, as it was mentioned, the game needs to be started with the "-eac_launcher" argument.

For me last drivers are very bad : the game doesnt start unless i uninstall reshade, and then SLI perf are worst than ever... :/
I switched back to 376.09
So SLI performance officially sucks as of the 1.09.154 update and/or latest driver. I was getting positive scaling, now I'm getting negative scaling. To make matters worse, this update now prevents temporal filtering from being enabled when SLI is in use, killing the biggest performance boost in the options menu. Old profile + new driver doesn't seem to help much either :-/
Tempted to make 3DMigoto fake out the game into thinking SLI is disabled so we can at least select temporal filtering again, but I need to work out how it is determining that (I tried the obvious call already)...
Maybe it's just me, but it really annoys me that they didn't bother bumping the minor version number for an update that is clearly not trivial as it broke so much. Also not happy that their patch notes are clearly incomplete. It's just bad software engineering practice.

378.49 No SLI: 28fps
376.33 SLI: 44fps
Nothing to do with the driver profile.
You previous patch 1.4 was a excellent.
This must be a real pain in a** to keep fixing would it be better to wait for 2 to 3 months after release before fixing a game.
Release v1.5 is up on the blog:
http://helixmod.blogspot.com.au/2017/01/watchdogs2.html
- Re-fixed shadows, etc. for WATCH_DOGS2 v1.09.154 update
- Fixed new rediculous shadow map size being stereoised (3DMigoto update)
- Disabled compatibility mode that was added in the 378.48 driver
- Scripted fixes applied to all (1.4GB!) shaders found in the patch.dat file, not just those seen used so far. Should fix Ansel in all circumstances along with any other shadows, reflections, etc. that might have been missing in any rare circumstances.
- Fix is now distributed as a 7z file to keep the filesize down (it would have been a 150MB zip file, compared to a 7MB 7z file. You can use 7-zip to extract it).
- Fixed road dirt decals (happened to notice they were out while taking an ultra-high convergence screen shot with the quad copter)

LZ4 is a pretty popular compression in game engines because it is *very* fast to decompress so it doesn't have much impact on loading times) ;-p

Btw/ Google finds a thread in Ubisoft Watch Dogs 2 forum, mentioning the patch on helixmod. But it cannot be found when searching the forum directly (stereoscopic or helixmod, for example). Clicking the Google link leads nowhere. Apparently it has been deleted.
