Under Windows 7 64Bit / Geforce 8800 Ultra / driver ver. 185.85 only a few refresh rates up to 85Hz are available for my CRT-display (22" Iiyama Vision Master Pro 514) which is capable of up to 200Hz depending on resolution. This is very unfortunate since I want to use Geforce 3D Vision which does work somehow with 85Hz but is very strainful for the eyes.
It does not matter which monitor driver I choose. Custom resolutions in Nvidia Driver with higher refresh rates reset always back to 85Hz.
There was one case when Windows 7 woke up from sleep mode and did not recognize my display (it said non-PnP) and only then I was offered refresh rates up to 170Hz in 1024x768 but only 256 Colors. As soon as I set it to the correect display driver I could not choose anything beyond 85Hz.
I do not know if this is a Windows Seven problem, Nvidia driver problem or both, but in other forums I saw people with similar configurations having the same problem with no solution yet. Anyone?
BTW I already tried display inf file modification and registry changes for the graphics adapter. It seems that the modern nvidia drivers cannot be influenced anymore by this.

I have had this same issue occur to me. It comes down to both the nvidia drivers and possibly a lack of decorated drivers for Windows 7 for your monitor.
It worked perfectly with drivers 182.50 but after upgrading to the latest drivers I cannot select the correct refresh rate. The nvidia drivers are limiting me to 85hz.
Have you tried decorating some drivers for your monitor? Do a google search on 64bit driver decoration and the try and find the latest driver for your monitor. Just bear in mind that driver decoration definitely does not work for me with drivers 185.85
