That's where this next mod comes in. The problem is, it conflicts with other mods, but currently is the only one working with GOTY 1.31
So you'll need to exit the game, and temporarily remove all the mods from the Mods Folder, drop this in there, start the game re-equip everything, then exit back out and swap the mods back.

5 much needed mods for Roach (your horse)
This first mod is absolutely awesome and makes the whole riding experience a 100% more enjoyable.
It turns Roach into a 4 legged off roading machine. At times, you can even jump off cliffs, it takes a little prodding.

This mod save a lot of time, when added up throughout the game. When you whistle, you are instantly placed in the saddle, ready to go.

The above mods are all currently installed and working well together, excluding the item level mod.
I tried a lot of mods and I can't specifically remember if I needed to use the "Unification Patch" or "Script Merger"

One thing I do know...some of these mods are not as simple as dragging and dropping the folder into your Witcher 3 "Mods Folder"
Some require that you also place or modify a few existing files. Like for instance, Immersive Cam requires that you alter the input ini, so that you can use keyboard/gamepad commands to adjust the camera. It also adds in-game menu features that let you adjust the various camera settings for each event, like combat, mounted, explore etc..
Skill Points per level, also has such a requirement. You then set it in you game's menu. NOTE: this can at times, reset itself, like when swapping out mods for the Item Level Mod, so be sure to check and reset it.
Some mods have a DLC file included, that needs to be placed appropriately.
To make it easier, you can use a Mod Manager, that is available. (use at your own risk)

There are a lot of other mods I tried, that gave me too many problems, so I'll list some of them that I remember
FriendlyUI
Better Torches
CrossBowDamageBoost
TradeMan
All Quest Objectives On Map
EDIT: I should say that the mods listed above, did not play well with the other mods that I wanted to use. By themselves or perhaps with only a few select mods, they might be perfectly fine.

Might I also humbly suggest these mods for a better experience:

The Witcher 3 HD reworked project
Increased LOD

Weather V4
Hairworks on everything but Geralt.
(Keep in mind that since early patches, all hairworks have been moved to the CPU as it performs better than the GPU - game was designed for consoles. This mod helps the CPU bottleneck bug in areas).

Unfortunately, I'm still using an old i7-950 with my 980ti. So I can't go crazy with anything that robs me of performance.
I have everything on Ultra with Hair Works off and I'm getting 60FPS, fairly consistent. I was going to wait until I built a new system, to play this. But decided not to wait.
BTW, The Witcher Trilogy is on sale cheap!! at Steam.

Includes The Witcher: Enhanced Edition Director's Cut,The Witcher 2: Assassins of Kings Enhanced Edition and The Witcher 3: Wild Hunt (note: 3 is not GOTY version)

That guy (Hoopermation VR) that uses helixmod/3Dmigoto for his VR videos, I wonder if he could play it using immersive cam. Perhaps add a toggle for first person to zoomed out/offset 3rd person.

I put an edit on my post.
It's just a matter of Friendly UI did not play well with a few of the other mods that I wanted to use.
This was the first game that I've modded like this and while I understood the basic principle of using Script manager and merging conflicts, I could never get it all working correctly.

Thanks for taking the time to post this up. I'm one of the probably very few whom has not yet played W3. Been meaning to, but kinda put off by the enormity of the adventure ahead, so being able to cut down the fat for a more streamlined approach might help to finally take the plunge.

Maybe I will post my mod list sometime this weekend, but I basically took the opposite approach D-man11 did.
I won't hate on you, but personally many of the things you list here would ruin the experience of the game for me, and I just consider a lot of it cheat mods. Instead of making it easier I was more interested in finding mods that would improve the gameplay rather than circumventing it. Still each to there own and this is the beauty of PC gaming. =)
My first play through was around 200 hours and is my all time favorite game I probably wouldn't have been saying this if I played it vanilla.
At this stage, I'll just say the only vital mods I recommend are the friendly UI and the improved camera mod. (especially awesome in 3dvision.) and if you are playing on pad the Insta signs improvement mod. I basically used all the graphics improvements under the sun as well especially the ones that removed those annoying so called "cinematic""on screen effects. Oh and that weight limit mod as I find those type of mechanics the opposite of fun.
I'm surprised you didn't use this.
https://www.nexusmods.com/witcher3/mods/546/
This simple mod allows you to have the skills active of any ability you put points into, this greatly expands build options and fixes the progression system in one fell swoop imo. (you can still slot abilities and get their bonuses, you just DONT NEED too.)
Also this does make the game alot easier as well but it's still very challenging on the hardest setting and I personally can't fathom why it's not how the game functions in vanilla.

Be careful with the "immersive camera" mod. It can break part of a story quest in the last DLC because it will affect your crossbow shooting distance, not letting you hit some targets in a minigame: https://steamcommunity.com/app/292030/discussions/0/350532795324387228/
It happened to me, but to be fair it was in 2016. I don't know if a newer update of the mod fixed it.
Nice to see some interrest for mods in here :)
Mods is what really makes some my favorite games so great, like FallOut and Skyrim but also the Witcher series has some amazing mods !
I'm not in a hurry when I play these games, you might say I use yet another approch than D-Man11 and Necropants !
My main goal is immersion both gameplay and graphic wise, that means away with all kind of screen- and cinema effects, I really hate those !!
Next I use all sorts of graphical improvements, like high res textures, improved water, weather and lights, but also some new nice models (which is essential in Bethesdas games, OMG the vanilla models is so ugly :)
I will definatelly try the "immersive camera" in my next Witcher playthrough, thats another gain with modding, it makes every playthrough different :)
D-Man11 please let us know if you experience any troubles with the DLC, which is probably the best DLC ever IMO !

I really disagree with many of your choices. I want to earn my gear and fight at an appropriate level, on the hardest difficulty. Starting with the best stuff and boosted stats makes the whole thing far too easy, and reduces the incentive to complete the interesting quests that grant you that gear.
But hey, you play the way you like, I'll play the way I like. My mod recommendations are mostly to take away annoyances and streamline the experience, while making minor graphical improvements:
Better torches: Allows you to climb/cast/etc while holding a torch.
All quest objectives on map
Autoloot
Better trophies: makes the stat bonuses trophies you claim from monster contracts a little more varied and theme-appropriate
Fast travel from anywhere
Friendly HUD: Allows you to configure the hud for a more immersive experience
High quality faces
Immersive Cam + alternative horse controls
Jump in shallow water
Lore friendly witchers (various graphical tweaks)
More blood
Nitpicker's patch (minor graphical fixes)
No fall damage
No dirty lens effect
Remove screen water droplets
Roach gallop dust
Ultra Goe
HD reworked project
Wolf Medallion
Unlimited sprint

Why have OP armor and weapons, if they can break? Fret not
No Durability Loss And Grandmaster Bonus Requires 1 Piece
https://www.nexusmods.com/witcher3/mods/2841
Now one thing with these high level armor and weapons. Anytime you are forced to dis-robe, like boxing matches, you will not be able to re-equip these items.
No Level Requirements for 1.31

An "absolute must have", is the mod that increases your weight carrying limit.
In the first two games, it seemed that my weight limit would always max out at the most inconvenient time. Piss on that!
Infinite Carry Capacity
https://www.nexusmods.com/witcher3/mods/792
This next mod is also very much needed.
No more dying from stupid little falls, in fact, you can now jump from the highest cliff and survive.
Note: there are some places that you are not meant to jump to and they result in insta death. This only happened twice to me.
No Fall Damage

This next mod is great, no more running back to the nearest signpost for fast travel

This next mod in over the top OP.
You can craft anything that you have a recipe/diagram for, anywhere, without mats.
So no more having to find the appropriate crafts man or having to run to your trunk/cache/bank.

Guards down-leveled and mortal

Note: it's always a good idea to save your game prior to engaging, usually they beat you down to nothing and walk away, but once I died.
A very nice mod that removes the annoying screen effects

Umm..why get 1 skill point per level, when you can get 6
Set Skill Points Per Level

Tired and annoyed of looting enemies/mobs that you killed?
Especially factoring in the mobs running off in every direction, then having to run to and fro collecting your bounty.
Well, no More!

Annoyed by Drowners pestering you while your trying to loot that underwater chest? Well not anymore!
This mod wildly increases the crossbow damage when used underwater.

Not into Gwent? Well to me it's tedious while trying to play the game.
Maybe on my next playthrough or perhaps, I'll buy and try the Gwent game. For now, insta wins, FTW
No time for Gwent

5 much needed mods for Roach (your horse)
This first mod is absolutely awesome and makes the whole riding experience a 100% more enjoyable.
It turns Roach into a 4 legged off roading machine. At times, you can even jump off cliffs, it takes a little prodding.
Spam the jump button, while working back and forth.
Better Roach Control
https://www.nexusmods.com/witcher3/mods/2000
This mod save a lot of time, when added up throughout the game. When you whistle, you are instantly placed in the saddle, ready to go.
Once in awhile, it doesn't work, this can happen if Roach is nearby, just run to him and mount as usual, other times, just sprint off for a moderate distance and then whistle.
InstaRouch

Easily win horse races or simply get to where you are going without having intermittent stamina slowdowns.
Infinite Roach Stamina

Geralt's banter with his horse is annoying, so to never hear him say "faster" again, use this modLeave Roach Alone - No More Horse Voice Commands

When using certain saddles, Roach's tail is messed up. Use this to keep it from happening.
Fix Roach's tail and mane

Out of all these mods.........the one mod that I would give up all the rest for is...
This awesome camera mod!! It allows the camera to zoom out or in, you can also adjust height and offset.
Being able to zoom out, has allowed me to play non-stop. with almost no motion sickness :)
Immersive Cam
https://www.nexusmods.com/witcher3/mods/689
I tried a lot of mods and I can't specifically remember if I needed to use the "Unification Patch" or "Script Merger"
Unification patch for 1.31 - 1.31GOTY - 1.31A game versions (UPatch)
https://www.nexusmods.com/witcher3/mods/2134
Script Merger
https://www.nexusmods.com/witcher3/mods/484
The Witcher 3 Mod Manager

FriendlyUI
Better Torches
CrossBowDamageBoost
TradeMan
All Quest Objectives On Map
EDIT: I should say that the mods listed above, did not play well with the other mods that I wanted to use. By themselves or perhaps with only a few select mods, they might be perfectly fine.
Naughty mods are available, search...

Nude All Girl
Wild Nudity Project
Background Girls Nude
New Vaginas
Brothel Swap
Might I also humbly suggest these mods for a better experience:
GFX:
The Witcher 3 HD reworked project
Increased LOD
Improved Sign Effects
Weather V4
Hairworks on everything but Geralt.
(Keep in mind that since early patches, all hairworks have been moved to the CPU as it performs better than the GPU - game was designed for consoles. This mod helps the CPU bottleneck bug in areas).
Less tedious Gameplay:
All quest objectives on Map.
Coloured Quest markers
Corrected sorting
Galloping in cities
Windows 10 64-bit, Intel 7700K @ 5.1GHz, 16GB 3600MHz CL15 DDR4 RAM, 2x GTX 1080 SLI, Asus Maximus IX Hero, Sound Blaster ZxR, PCIe Quad SSD, Oculus Rift CV1, DLP Link PGD-150 glasses, ViewSonic PJD6531w 3D DLP Projector @ 1280x800 120Hz native / 2560x1600 120Hz DSR 3D Gaming.
BTW, The Witcher Trilogy is on sale cheap!! at Steam.

It works well and for immersion, it's a must have. At home, he worked well.
ASUS VG248QE - 3D Vision 2 - GTX 1080 - i7 7700 - 16GB RAM - WIN 10
It's just a matter of Friendly UI did not play well with a few of the other mods that I wanted to use.
This was the first game that I've modded like this and while I understood the basic principle of using Script manager and merging conflicts, I could never get it all working correctly.
3D Gaming Rig: CPU: i7 7700K @ 4.9Ghz | Mobo: Asus Maximus Hero VIII | RAM: Corsair Dominator 16GB | GPU: 2 x GTX 1080 Ti SLI | 3xSSDs for OS and Apps, 2 x HDD's for 11GB storage | PSU: Seasonic X-1250 M2| Case: Corsair C70 | Cooling: Corsair H115i Hydro cooler | Displays: Asus PG278QR, BenQ XL2420TX & BenQ HT1075 | OS: Windows 10 Pro + Windows 7 dual boot

It happened to me, but to be fair it was in 2016. I don't know if a newer update of the mod fixed it.

Win7 64bit Pro
CPU: 4790K 4.8 GHZ
GPU: Aurus 1080 TI 2.08 GHZ - 100% Watercooled !
Monitor: Asus PG278QR
And lots of ram and HD's ;)
But hey, you play the way you like, I'll play the way I like. My mod recommendations are mostly to take away annoyances and streamline the experience, while making minor graphical improvements:
Better torches: Allows you to climb/cast/etc while holding a torch.
All quest objectives on map
Autoloot
Better trophies: makes the stat bonuses trophies you claim from monster contracts a little more varied and theme-appropriate
Fast travel from anywhere
Friendly HUD: Allows you to configure the hud for a more immersive experience
High quality faces
Immersive Cam + alternative horse controls
Jump in shallow water
Lore friendly witchers (various graphical tweaks)
More blood
Nitpicker's patch (minor graphical fixes)
No fall damage
No dirty lens effect
Remove screen water droplets
Roach gallop dust
Ultra Goe
HD reworked project
Wolf Medallion
Unlimited sprint
