The SBS/TB and eye flip feature is enabled by default in the current 3Dmigoto. So any fixes made with the current version will automatically work and not need any workarounds.
For an older fix like Witcher3, we were using a 3Dmigoto that predates this addition. That's the time when you might want to update to the latest version.
As a general rule the d3dx.ini that comes in the latest version is always our newest and best template. You are better off to start with that, and merge in the game specific changes to it.

Thanks Bo3b, so it should be simpler for more recent games, and I think I did it right for witcher 3. Is it normal to hear some beep sounds (like 4-5 beeps) when I launch the game?

Try to uninstall the fix with uninstall.bat provided with the fix itself. Then try just copying the file attached in this post (you have to download, then remove .jpg from its name and unpack it) into your w3 x64 folder. It's the files I have in it and for me it works. Let me know

Thanks for helping me troubleshoot. Tested your file this morning and successfully got TaB 3D to work on my projector! Progress! Based on my comparisons with 3dtvplay at 720p (downsampled from 1080) I think the PQ is better with TaB mode with a few caveats: there is greater aliasing / shimmering / pixelation effect on edges. At 720p with downsampling the picture is smoother but also "soft" covering up details. I will definitely prefer playing in TaB 3D when available.
Now for the road bumps: The default 3D switch toggle is F11. This key is also set in the 3D fix to disable HBAO+ in case of 3D glitches. I switched the toggle to F12 to prevent the HBAO+ from being toggled off as I think this has a big impact to the picture. Just a note to make sure your not switching off HBAO+ unintentionally.
Second - since installing your files (w3TABfix.rar) my sweetFX no longer enables which previously worked with the 3D fix before. This may have been the root of why it wasn't working for me before. I uninstalled the fix and noted that my SweetFX would load again but only if d3dcompiler_46.dll was in the folder. I also have a d3dcompiler_47.dll in the folder, perhaps from an older version of the fix? Anyways there seems to be a conflict now and I'm not sure how to get both to cooperate. Personally, I think the Sweetfx/Reshade makes a big improvement to the picture.

Typically when running 3Dmigoto and reshade, you need to make things play nice, you can check these threads.
So... you are not understanding the principle for how these dlls are loaded. We rename them sometimes, but for very specific purposes. In general, renaming is not going to work.
If for example both reshade and 3Dmigoto use d3d11.dll as their library, that means they are overriding the system d3d11.dll to do our modding. You can't have two files with the same name, so one would need to be renamed, then proxyed from the other. It's a daisy-chain load, telling 3Dmigoto for example to load the renamed reshade after it loads itself. That's all the renaming is for.
In this case, since reshade uses dxgi.dll and 3Dmigoto does not, that means there is no conflict at the name level, and thus no need for renaming.
Since it doesn't work with them both installed, they have some incompatibility.
Make sure absolutely NO overlays are in use. No Steam, no shadowplay, no afterburner, no nothing. ALL of those ALSO override d3d11.dll, and can easily cause problems.
If that still isn't it, try two more things-
1) an old version of 3Dmigoto that has no DXGI support, and thus no conflict. https://github.com/bo3b/3Dmigoto/releases/tag/1.0.1
2) Flugan's wrapper. It uses hooking, not wrapping, and might be able to load the 3D fix without having a conflict with reshade.
I just repackaged the fix with the latest template from 3Dmigoto so that SBS and TB will work, but I see there is an F11 conflict with the fix.
F11 is not a good choice for this Disable HBAO+, because it's also used for Ctrl-Alt-F11.
I haven't played enough Witcher3 to know, so question: what is the right key for the Disable HBAO+?
Also, is it still necessary/useful?
I'll update the fix with whatever you recommend.
FWIW, I was not impressed by ReShade in Witcher 3.
All it seems to do is remove the depth fog, increase the contrast, add a bit of blue tint. I have seen the comparison videos, and even tested it myself. I wanted to like it but I just kept getting the feeling of playing an older game with in installed.
Removing ReShade (and even the turbo lighting mod), IMO, the picture quality is overall more immersive and better.
I did have to increase the gamma to 75% from default 50% though.

I've been using this profile (http://sfx.thelazy.net/games/preset/3707/) which I found was a little more subtle than the most popular reshade+sweetfx profiles. I found it removes the soft yellow haze and increases the sharpness to help appreciate the textures. Looks more like original trailer.

Ah, to each his own ;-)
The haze is deliberate, to emulate dust in dry parts of the earth, and sharpness can be set to off - medium - high in the post processing options. Like you, I too like high sharpness, which looks amazing with 4x DSR - try it out if you haven't already!
The main thing is that you personally like what you have! :)

Thanks for the update Bo3b. Sorry I'can't answer your question cause I play with a razer tartarus and a logitech G600. Since I customize almost every button I almost never used F11 etc. Maybe someone who plays with a more standard setup can give a better answer.
As to reshade, even if this is not a w3 topic, i used to play with this: http://www.nexusmods.com/witcher3/mods/1189/?
To me it looks muuuuuch better than the original in almost every location. Pity it's not being updated anymore, I hope it is still as good as it was. I suggest you try it!
It's good also with this for darker nights: http://www.nexusmods.com/witcher3/mods/1305/?
