Glad you were able to fix your driver related issue and get the fix working! Sorry, but I don't bother with playing in CM mode, so I can't really help you with that, but hopefully someone else might be able to.

The best answer I can give you is "because the game has it's share of bugs". I've had my Xbox controller stop working on a few occasions, and what works for me is to go into Device Manager, uninstall it. Reinstall it by plugging back in. Reboot, and I'm usually fine from there.
One thing I figured I'd mention (since no one else seems to have noticed) is that I've discovered that my fix for the 2D geometry actually breaks shadows... or, more specifically, without the 2D geometry fix in place, those objects do cast shadows correctly, but then when applying the fix, although it's adjusting their positions to the correct locations to us, the game thinks their original positions are correct, so when they get shifted so do their shadows, but by 100 fold and they actually go way out of our field of view. Been trying to determine a viable fix for that (found something, but it's inconsisted and glitchy), but that's not really your typical type of issue so it's not like fixing a standard broken effect, I think it's more like fixing a driver bug through 3DMigoto's more advanced features, which is more the area of DarkStarSword's expertise, so I'll try scouring his Github for a possible relevant example.
Another thing that got me stumped is adding in a stereo crosshair and auto-depth for in game icons, so we may be stuck with just static HUD elements, but I'll at least add layers (ie. minimap at less depth than the crosshair, for example). Probably post the fix to the blog by the end of the week, and whatever gets resolved by then can be considered final.

You have already tried using other StereoTextureEnable settings, right? To this date I haven't seen any game with proper polygonal geometry in 2D unless everything else was in 2D too, aside from turf effects in FFXV which render in one eye.
What's the problem with the dynamic HUD? Let's see if some of these things help:
- In Turok 2 (still work in progress) I needed to set "max_copies_per_frame = 4" for my "ResourceDepthBuffer". Otherwise it didn't work at all or it would get stuck at certain depth levels. In all the other games I use "max_copies_per_frame = 1".
- Different games needed different stereoization formula multipliers. Yakuza 0 needs x10, other games x5, x1, Skyrim SE needs a small decimal, etc. If your multiplier is too low, you'll notice it at very high convergence levels (the crosshair depth will vary very slightly).
- Are you getting a working depth buffer? "oD unless_null" hasn't failed me yet (in the few games I made a dynamic HUD). I didn't have problems getting it from the HUD shader directly yet.
- When hunting in Yakuza 0, realoading shaders, making ini changes, etc, some actions made the HUD stuck in one position, as if the depth buffer froze. But with normal gameplay it didn't happen.
- In the games I have, I never needed a depth range other than a small decimal (can't remember the number of zeroes. Check my Turok fix for example) as minimum and 1 as maximum.
- Use at least a few thousand of samples just in case. Reduce them after you're finished if it works fine with a lower number.
- As you probably know (most surely you told me this yourself, but this can work as advice for others), the easiest way to edit the crosshair code is using it directly inside the shader, rather than using an external file.

The DJ-RK fix works but I have no access to the xbox 360 controller anymore

I tested the driver Nvidia 399.07 of 27.08, the fix works correctly (at first glance). May be a slight gain over the 398.36 driver for me... to confirm.
I also recommend alpha reshade preset (see nexus mod manager) ... visually very pretty with real undelivered colors :)

It's a proper 3D Vision fix. Pretty much any time you need to download and install a fix it's for 3D Vision. CM mode "fixes" usually just have you setting the Nvidia driver profile for the game to a different one that is known to have decent CM mode settings that will also work for the game in question.

btw i can see you have an sli, is the sli worth in monster hunter ?
i'm still hesitating to make a 2080 ti sli (3D 4K is very demanding...)

Probably a reply to a dead/old thread, but this is an issue I just started running into after I updated to the most recent Nvidia Drivers, v411.70. Since updating, portions of my screen flicker (various segments and portions) arbitrarily. Sometimes in Astera, randomly throughout any hunt I do the screen will flicker randomly. Framerate stays consistent despite the flickering. I turned the graphics down at one point and it mitigated the flickering but only slightly. I can be standing in one spot in Astera and it'll start flickering for no readily apparent reason. My previous driver update was I think v386.73 and it ran MHW with zero problems. I tried rolling back the drivers but I couldn't for some reason and I never uninstalled the previous drivers so I don't know why I can't roll back. Any ideas?

Nope, not for MH:W. The only working SLI profiles that people have come up with cause crashes, flickering, and some graphics corruption. Might be tolerable for some people, particularly those that already have SLI, but it's definitely not good enough to make a recommendation for getting SLI.

Only the most fundamental suggestion: Have you tried a full uninstall using DDU (Display Driver Uninstaller) before rolling back? That should do the trick if you haven't. Simply uninstalling and reinstalling the driver through the control panel doesn't completely remove all driver files, DDU does. Good luck!

One thing I figured I'd mention (since no one else seems to have noticed) is that I've discovered that my fix for the 2D geometry actually breaks shadows... or, more specifically, without the 2D geometry fix in place, those objects do cast shadows correctly, but then when applying the fix, although it's adjusting their positions to the correct locations to us, the game thinks their original positions are correct, so when they get shifted so do their shadows, but by 100 fold and they actually go way out of our field of view. Been trying to determine a viable fix for that (found something, but it's inconsisted and glitchy), but that's not really your typical type of issue so it's not like fixing a standard broken effect, I think it's more like fixing a driver bug through 3DMigoto's more advanced features, which is more the area of DarkStarSword's expertise, so I'll try scouring his Github for a possible relevant example.
Another thing that got me stumped is adding in a stereo crosshair and auto-depth for in game icons, so we may be stuck with just static HUD elements, but I'll at least add layers (ie. minimap at less depth than the crosshair, for example). Probably post the fix to the blog by the end of the week, and whatever gets resolved by then can be considered final.

What's the problem with the dynamic HUD? Let's see if some of these things help:
- In Turok 2 (still work in progress) I needed to set "max_copies_per_frame = 4" for my "ResourceDepthBuffer". Otherwise it didn't work at all or it would get stuck at certain depth levels. In all the other games I use "max_copies_per_frame = 1".
- Different games needed different stereoization formula multipliers. Yakuza 0 needs x10, other games x5, x1, Skyrim SE needs a small decimal, etc. If your multiplier is too low, you'll notice it at very high convergence levels (the crosshair depth will vary very slightly).
- Are you getting a working depth buffer? "oD unless_null" hasn't failed me yet (in the few games I made a dynamic HUD). I didn't have problems getting it from the HUD shader directly yet.
- When hunting in Yakuza 0, realoading shaders, making ini changes, etc, some actions made the HUD stuck in one position, as if the depth buffer froze. But with normal gameplay it didn't happen.
- In the games I have, I never needed a depth range other than a small decimal (can't remember the number of zeroes. Check my Turok fix for example) as minimum and 1 as maximum.
- Use at least a few thousand of samples just in case. Reduce them after you're finished if it works fine with a lower number.
- As you probably know (most surely you told me this yourself, but this can work as advice for others), the easiest way to edit the crosshair code is using it directly inside the shader, rather than using an external file.

as soon as I remove the fix and come back in 3d CM the handle works perfectly
I tested the driver Nvidia 399.07 of 27.08, the fix works correctly (at first glance). May be a slight gain over the 398.36 driver for me... to confirm.
I also recommend alpha reshade preset (see nexus mod manager) ... visually very pretty with real undelivered colors :)

Nope, not for MH:W. The only working SLI profiles that people have come up with cause crashes, flickering, and some graphics corruption. Might be tolerable for some people, particularly those that already have SLI, but it's definitely not good enough to make a recommendation for getting SLI.
Only the most fundamental suggestion: Have you tried a full uninstall using DDU (Display Driver Uninstaller) before rolling back? That should do the trick if you haven't. Simply uninstalling and reinstalling the driver through the control panel doesn't completely remove all driver files, DDU does. Good luck!
