I'll try to help tomorrow, as my day is ending just now. If nothing works as is, you'll have to use the old method of using Special K to force fullscreen (booting games is more painful, but ingame the experience is basically the same).

I believe I am. Checked and I am on Windows 10 pro x64, version 10.0.16299 (think that is Redstone 3, Fall Creators Update). Thanks in advance for helping with this, look forward to figuring it out. Can't wait to check out GOW 2 in 3d, right before GOW 4 comes out.

Do you have a G-Sync monitor? In my case 3D Vision started to work at the same time as PCSX2 started triggering G-Sync without Special K. Months before that, it didn't.
Have you tried PCSX2 with Special K already (using that "d3d9.dll" and "d3d9.ini")? It's the file linked in the last installation step: https://s3.amazonaws.com/masterotaku/PCSX2/SpecialK_for_PCSX2.7z

Yes, I have a G-Sync 3d vision monitor (Dell S2716DG). And thanks for the latest attachment, I guess I needed SpecialK for my setup (not even sure what it does, will have to look into that later). Unzipped the files into the root of the PCSX2 dir. Thought I might have to switch to DX9 instead of DX11, but it worked with DX11 now that I added these files. Hit Alt + Enter and tweaked the video plugin, went full screen again and 3d vision turned on. After doing some modification within GOW2 with the hotkeys (b + h + g), got 3d to look great. Awesome stuff!!! Thanks for the help! Nice work masterotaku!!! Really cool way to play some old school PS2 games.

Maybe the eyes are swapped for some reason? Can you turn the T-3 upside down and hold it to your face and see normal 3D? (of course, it will be upside down)

If you open "d3dx.ini" and change "hunting=0" to "hunting=1", do you see the hunting overlay after you start the emulator and a game?
Also, why are you using the Assassins Creed 3 profile?

So you DON'T see the overlay saying your separation and convergence, and shader hunting? If not, that means that you didn't install "d3d11_fix.sdb" correctly with the ".bat" file.

Make sure you open a CMD window with admin rights. It should show a message.
