Just tested this again, and it was working correctly here. No problems with graphics, still 3D Vision Ready.
Running on Win10 1803, Driver 419.67, Steam version. I used 3DFM to setup and launch it.
For your scenario, next best suggestion would be a clean driver install using DDU. The missing grass pieces suggest that driver might be corrupted, or at least the profile is wrong.

I'm running Win10 1809, driver 418.91 and 3DFixManager v1.7 (profiles v2.28): it's probably better to install your version of nVIDIA drivers while I'm at it... I'll proceed with a clean installation in the next 24 hours and post back the results (just in case someone will encounter similar problems in the future).

OMG That did it... THAAAAAANKSSSS!!!! :) :) :) :)
The game now run FLAWLESSLY, not even a remote ghost effect... I am still baffled by the amount of issues which can be solved by just cleaning up everything and reinstalling from scratch... This solution is definitely getting up there with "did you try turning it off and on again?" :D
Again, thanks a lot bo3b and DarkStarSword - now if you'll excuse me I'll head back to Afghanistan as there are still many bad dudes to convert to my cause... :)
