I switch back and forth between 3DTV Play and 3D Vision often. Let's say you have 3DTV Play installed. Create a restore point under 'system protection' label it '3DTV Play restore' or something you will remember. Next, install rollermod.inf. and install cd1.45 to get 3D Vision. To return to 3DTV Play, do a system restore using the restore point you just created. Remember, restore points get auto deleted by windows (oldest first), so make sure you configure your system restore capacity to a large value. I believe default is 3%, I changed mine to 36%. For saftey, create a brand new restore point everytime you re-install rollermod.inf

Not sure, I connect my gtx480 directly to HDMI1 input on my Samsung plasma.
