Well, ain't this interesting...
Put my old 560TI back in since it supports much older drivers.
Firstly with the 355.60 drivers, exactly the same stuttering as my GTX970.
So swapped to the ancient 320.49 drivers. Stuttering is 95% better, and totally playable. There's still the tiniest of stutters here and there and sometimes in the same places as previously but it's a single frame stutter rather than dropping to 5 fps for 0.5 seconds.
What would be useful here is if someone else with Dark Souls installed (it's quite a small download btw, 3.7GB) could confirm that it stutters with all the 970-compatible-era drivers. I know o-leif had it working last year but he has double the GPU power I have.
You don't even have to install DSFix or the helix mod to test it btw. I can upload my save somewhere if it'll help and direct you to a spot that stutters a lot.

Thanks. I figured I'd give those nvidia inspector changes a try, but they didn't impact the stuttering. I know you weren't necessarily suggesting they would.
The stuttering isn't constant and it's not dreadfully bad, but it's enough to annoy, and it's disappointing to see it on my 970 when it wasn't there on a much slower 560TI. Since no one else is getting it, I'm just going to assume it's some minor thing that's gone wrong with my OS install or something.
edit: Actually after playing last night for 4-5 hrs, I'd say the stuttering is close to being bad enough to ruin the whole experience.

Switched to Win10 and a lot of the stuttering is gone. Probably not a Win7 issue though, more likely my old OS install was screwed up somehow.
It's at least playable now. There are still some areas that cause bad stuttering though. For instance, the stairs down to Capra Demon (once the shortcut is open) are really bad.

If you don't use Dsfix, you are absolutely crazy. It's absolutely mandatory to fix this game to a playable state and helps make along with the helix fix one of the best games in 3d.

Just for the heck of it I decided to give this a try since I have both win7 and 10 on dual boot. The problem that you are seeing on Win10 seems to be a problem with 3D Vision and this game. The right eye cut out happens with just 3D enabled with no fixes applied at all. If you shut off 3D the problem goes away. I would stick with Win7 for older DX9 games for the time being maybe. I will say, this maybe an issue with newer drivers, I seem to remember getting this game to work on win10 with dsfix and the helix fix near the end of last year.
I will try some older drivers and report back.
Edit: Alright, well this is really weird. I did end up getting this game to work in win10 using the newest version of dsfix and the helix fix both. Once you copy both the dsfix and helix contents to the data folder. Edit the dsfix.ini file, change the internal rendering resolution of the game to at least 1080p or greater. Leave the downscaling alone, leave it at 0. What is odd, the green text from NVidia (the game rating does not show up, even it you hit crt-alt-F11 or w/e key you have it bound to). However, if you start up the game, 3D is on, DSFix is working and the helixmod fix is working as well. No right eye cut out either! Make sure you disable blur and anti aliasing in game as well for helixmod fix to work properly.
