I also have the game on GoG.
On Desktop: I installed via Galaxy Client. Made a "Repair" check and I think it downloaded some new files? (Wasn't paying attention). Problem went away.
On Laptop: I installed it via the normal installers :( So I can't "repair".
Because this game was on my laptop since version 1.01 and I kept on patching it I wonder if one of the patches doen;t have a "bug" during the patch process ;))
In any case I think is best if you just try to re-download it using Galaxy;) I expect the problem will go away then;)

Note that repairing the files did stop my crashes that were actually in game. It has not stopped my occasional crash as I alt-tab back into the game from my desktop, which happens about 5% of the time. It does appear to be 3D related, as it happens exactly when the view is switching back to 3D following your alt-tab the other 95% of the time, and the frozen screen you get in the crash shows the two stereo images simultaneously albeit in 2D (with one Geralt image having an almost bald chopped off bun on top of his head, and the other image with the hair, heh. That stereo scene is actually visible for just a moment during the switch back to 3D following the alt-tab when it all works correctly, it's just that 5% of the time it can't complete the last step in the process, hangs and crashes.

Just a quick update on this:
It is possible to install Galaxy and select the "Import Folder" option. It will import the Witcher 3 folder, make some diffs and etc, patch/fix the files. It takes around 15 minutes or so BUT YOU DON'T HAVE TO RE-DOWNLOAD the game again;))
I fixed my Laptop installation like this in the weekend;)

I've been having one problem and wanted to see if anyone can take a look and see what it might be. Faraway objects like mountains sometimes have almost like a refection or something to them. I attached a few pictures and my save game file if anyone has a Chance to take a look at them. That's the only thing that seems broken. Everything else looks perfect.

Never seen that before - from the screenshots it almost looks like tessellation is only working in one eye (but I'm not sure if the distant mountains use that, otherwise it might just be a halo). Does it happen if you disable SLI?

SOrry to get off topic. But i am determined to get Nvidia's, Project Red's and anyone else's attention to hopefully finally get this anoying yet looks to be relatively simple BUG to get fixed!
THIS has been a problem since the release of this game! and multiple threads have been created on RED Project's Website and Nvidia's Forum website regarding the game since its release!
THIS CAN NO LONGER BE IGNORED!!
I am so Dissapointed that this problem has been ignored this entire time!
I pay BIG money to Nvidia and EVGA to have bought 3 way SLI GTX Titan X's

I've looked at the savegame but everything looks perfectly fine for me... What are your in-game settings... Maybe the game uses a shader I don't in my settings:

Thank's for taking a look for me. I've tried changing to different settings in game but still have the problem. I have 2 GTX 980 Ti's in sli and have tried different drivers but that didn't solve it either. I actually just finished the main quest with just over 100 hours a few days ago and will be playing hearts of stone soon and thought I would see if I could find help to fix this but its not that bad and I've got used to it. I've tried to play a little of Fallout 4 but its hard to get in to after how beautiful the graphics and scenery look in the witcher. I think this game spoiled us. :)
