First thing i do is turn off v-sync and FXAA when i load up a new game. Strange that tearing never bothers me and i don't even notice it outside of a couple of games. when i do notice it, its bad sometimes and the tear line is right in my face for long periods of time, but very rare. Strange.
Fallout 4 tears the screen like it's ripping butthole. You're right, some games are insanely bad.

For me with Active 3D monitor Vsync is mostly on, some games cause dizziness, some cause disconfort while Vsync is off. With Passive 3D Vsync is mostly off, except when SBS or T&B used; especially with T&B Vsync is always on, Vsync with SBS depends on the game. While playing games in 2D with 100-144hz, Vsync is always off. I mostly don't care about screen tearing, but in very rare situations Vsync is on.

So the custom resolution I should try is 1920 by 1085?
You should use something lower than your native resolution. And by "+5 Vertical Total" I mean in the timings window. Like this one that I found on the internet: Create for example 1920x1078 under "Display mode", and then in the "Total Pixels" "Vertical" input box (1149 in that screenshot) use something +5 higher than what it automatically suggests you. In my case it was for example 1530 instead of 1525. Try creating the custom resolution while G-Sync is disabled, just in case. I didn't try doing it with it enabled, I think (or maybe I did).
So unfortunately when I created this custom resolution I wasnt able to enable ULMB in it for some reason.

I pretty much set NCP to force adaptive mode and leave it. If I do not I get this really weird disorienting effect in alot of games. btw I cannot stand tearing of any kind in 3d, and I play competitive games too.. I just get used ot the input delay. As long as it's consistent its usually not bad to add a couple of frame. it's when it's variable when it becomes a nightmare.
