Seems like you've done some good legwork there to try to narrow the crash down.
Enable the full logging for 3Dmigoto, and I'll take a look at the log to see if I can see anything.

Happy to know that Helifax, cause it means it's likely a problem on "my side" and I just have to find it. Problem is it keeps crashimg on my system, tried again now, I'm sure I don't have any overlays, even uninstalled them to be sure, Steam is offline mode with both general overlays and W3 overlays disabled, my taskbar is almost empty, task manager also. No mods, fresh install. No overclock, nothing "custom". I really don't know what to do. Those crashes in those places are unavoidable with 1.21 (Helixmod fix I mean, not game version), never rhappen with 1.10. My samsung SSD has "rapid mode" active, could it be an issue? Do you have any other suggestion for me to check?
Maybe this could be of help: for example in Kaer Morhen, I teleport in front of the castle, I can stay there all the time I want looking at the bridge without crashes, but if I turn like 140° on the left or right... Boom. As if in that area is trying to render something that make me crash. Also Crookback Bog, I teleport, ok if I look forward, crashes guarantee if I turn back.

No, you don't need GFE installed;)
While I use it for the FPS counter, it doesn't affect this game. But I don't have "Instant Reply" activated.
I don't know exactly what to tell you to try there:(

The fix relies on an older version of the wrapper. I can update it to the latest version but the crossbow crosshair will not be dynamic. Still if it will work for you I guess it will be the best solution.
Before though, can you do one test for me?
- Add the FIX
- Delete the "ShaderFixes" folder.
This way only the wrapper will be loaded but without any shader fixing. See if it still crashes like this;)

You edit the d3dx.ini file that comes with the fix, those commands are at the top of the file, with 0's instead of ones. 0 means off, 1 means on. If you turn those three settings on, it will generate a d3d11.log file with extensive debug data.
This may no longer be necessary though. Since the crash doesn't happen with the ShaderFixes being deleted, that suggests one of the shader files is causing a problem. That would be unusual, but not unprecedented.
If you want to [i]really[/i] help, do a binary search (half in, half out) of the ShaderFixes files to narrow down which one is making it crash for you.

The very powerful and the very stupid have one thing in common. Instead of altering their views to fit the facts, they alter the facts to fit their views ... which can be very uncomfortable if you happen to be one of the facts that needs altering.

You can try also deleting only the .BIN file (don't delete the .txt file)....so when you first launch the game all BIN files are generated again in your computer.

As to what Bo3b asked, could anybody give me a hint on what I have to do? Where do I have to write those lines of code? Sorry but this is clearly not my area of expertise... I'm just a humble gamer :)
Cool thanks for the test! I'll try to update the fix to use the latest version of 3DMigoto in the following days (but probably will end up doing it in the weekend due to limited available time).

Google is your friend for this sort of thing. Lots of discussions.
You edit the d3dx.ini file that comes with the fix, those commands are at the top of the file, with 0's instead of ones. 0 means off, 1 means on. If you turn those three settings on, it will generate a d3d11.log file with extensive debug data.
This may no longer be necessary though. Since the crash doesn't happen with the ShaderFixes being deleted, that suggests one of the shader files is causing a problem. That would be unusual, but not unprecedented.
If you want to really help, do a binary search (half in, half out) of the ShaderFixes files to narrow down which one is making it crash for you.

Just out of curiosity, is it possible to know which shader is that? I mean, what it does in the game.

You can try also deleting only the .BIN file (don't delete the .txt file)....so when you first launch the game all BIN files are generated again in your computer.

Binary searching just means splitting the list in two over and over again until you find the one problem file. All I did was move half the shaders to another folder. That crashed so I moved half the remaining shaders to yet another folder, and so on. If it didn't crash then I took the previous folder (which must have had the crashing file) and used half of that. The txt and bin files obviously went together so I didn't split those apart. I've got Witcher 3 on an SSD so it didn't take all that long to narrow down.
