You're right Helifax, but in my case I can't download any of the fixes with Firefox !
I know it can be solved somehow in the browser, I just didn't bother to do so..
With IE or Chrome there's no problems !
I assumed that Arni1984 had the same issue, thus it looks exactly the same way as described here..
This mod removes the "Talk" icon from above the head. It still shows up when you approach someone but you have to be right next to him. Won`t pop out when you walking about as it use to be. So much better now.

Nop. It all should be good. Check and install properly your fix or drivers.

That's so much better, thanks for bringing it up SKAUT :)
Yeah, thanks SKAUT. that thing really improves things. Anyone else got any good mods they use?

My used mods:
What I consider Mandatory:
1. Immersive Camera + Immersive Meditation
http://www.nexusmods.com/witcher3/mods/689/?
2. FRIENDLY HUD (allows you to control HUD in various ways). It also allows you to disable the "talk icon". It allows you to make the whole HUD context dependent:)
http://www.nexusmods.com/witcher3/mods/365/?
3. Lore Friendly Witchers:
http://www.nexusmods.com/witcher3/mods/390/?
(THIS is how Witchers should look like - Like it was in Witcher 1 and Witcher 2!)
4. STLM (Super Turbo Lighting Mode)
http://www.nexusmods.com/witcher3/mods/657/?
5. Lore Friendly Ciri:
http://www.nexusmods.com/witcher3/mods/685/?
(This is a personal preference)
Optional and nice to try:
1. Get Your Menu Back (allows you to select what MAIN MENU the game shows)
http://www.nexusmods.com/witcher3/mods/1022/?
(I normally use the Default Menu in main game and Expansions Menus when I play the expansions, but this is RolePlay:) Kinda Like Witcher 2 did for Different Chapters ^_^).
2. Hair on everything except Geralt (doesn't affect Beards or animals) - I hate the "noodle" hair.
http://www.nexusmods.com/witcher3/mods/165/?
3. Better Trophies:
http://www.nexusmods.com/witcher3/mods/1006/?
4. Better Merchants with Fari Sale (last version of the mod for 1.21 is in the "Discussion" section)
http://www.nexusmods.com/witcher3/mods/6/?

Thanks Helifax. Ciri mod is great but that Friendly HUD never worked for me that's why I was using that simple one for the "Talk" issue.
Also I would suggest mentioned long time before but greatly improved HD Rework Project. It improves all textures to insane quality without changing visual style and design. There is 2K and 4K textures resolution available for Rocks, Trees, Roofs, Doors.....and lots of other elements of environment.
Really nice addition. Definitely recommended.
Edit:
What STLM (Super Turbo Lighting Mode) really do ? Is it only adjustment to global illumination or something like SweetFX ?

STLM: It affects how the lighting system works (something that SweetFX does with post-processing). So you can achieve something like SweetFX). I find it pretty nice because I don't have the FPS impact of SweetFX as I play the game in 3D Surround;)
What I failed to mention is that you will also require
Witcher Script Merger:
http://www.nexusmods.com/witcher3/mods/484/?
To merge the scripts if you encounter errors (Friendly UI, wink, wink)

First of all: So many thanks for the quick 1.21 Fix, Toussant is such a beauty;)
I go with this Reshade... works flawlessly even with MS AB Overlay, nearly perfect Contrast and no `Fog Wall`. no Colour changes, because imo in this case CD Red did a good job
