I'm now at the endboss and discovered that he has a distortion effect around his sword making it unplayble in 3D :( I have attached a savegame (BTW could You use the savegame I have posted above for the Darkmoon Tomb?) and several PS and VS. Two PS with the comment "boss fog disabled for hunting" are just used to disable the fog gate so You can observe the issue before entering the fight and getting attacked. The other PS and VS seem to be responsible for the issue. At least in the area of the endboss they didn't seem to affect other effects. But I'm not sure how specific they are.
Maybe it's the same issue You observed with the Watchers of the Abyss...

First of all, I want to say thanks for all the work on this patch. I downloaded the latest file and it looks amazing now, as good as the Helix Fix for Dark Souls Prepare to Die edition.
The only complaint I have it that the game seems to crash a lot more when 3d vision is enabled - and not simple crashes to the desktop but system hangs with the speakers blaring that require an 8 second power button press to overcome. I don't think this is down to the 3d vision fix - it's probably down to a lot of things including the Nvidia drivers but I do wonder why it only crashes like this when playing with 3d vision enabled? Has anyone else experienced more crashes with 3d?
Also, whenever I first enable 3d vision the colours are all wrong (blues and browns) and I have to ALT Tab to fix it.

You have multiple issues there from the sound of it:
1. System Crashes: The thing that you describe is a Driver Kernel Crash. A lot of people are reporting crashes like these in the Nvidia drivers for quite a few months now. While I don't get them (very very very rare I get one but is not in this game, just general) they have nothing to do with the 3D Fix or 3DMigoto.
2. Game crashes: The game has a HUGE MEMORY LEAK in it! And by huge I mean HUGE! I am playing in 3D Surround + SLI and because I run triple-wide monitors the resource usage is also increased. If I travel alot from bonefire to bonefire my RAM increases up to 10GB ALONE for the game and keeps on increasing UNTIL the game crashes.
3. FPS related issues in 3D Vision: Also another interesting thing I noticed on my 3D Surround (but don't think is present in single screen) is that I need to disable 3D (ctrl+t) before going to a loading screen. I enabled it back after the game loaded. This needs to be repeated FOR every Single Loading screen.
Failing to do this my FPS is lowered to 11FPS from 60FPS. With the above workaround is always at 60FPS.
Again shows there is something wrong with the game.
4. For the colour problem, did you installed the latest WHQL drivers? Or at least import the Custom profile:
https://forums.geforce.com/default/topic/925628/3d-vision/dark-souls-iii-3d-vision/5/
The problem should go away.

I have 16GB DDR4 and it's crashing in a single area after a short amount of play time - so I don't think it's the memory leak. I'm not really getting any FPS issues either - it is playing fine and then crash, out of the blue.
All I change in the d3dx configuration settings is put the HUD to 0.0.
I already have the latest drivers but just imported the custom profile - so we'll see if that helps.

For the colour problem, did you installed the latest WHQL drivers? Or at least import the Custom profile:
This problem happens when using HDMI checkerboard or EDID override some games if you press the Ctrl-f11 key will correct the colors after the colors are correct save the profile.
This works with Dark Souls 2 (direct x11) and GTA5.
The only solution I find for Dark Souls 3 was to alt-tab.
I can live with the ALT-Tab it is sure a good fix.

That is a memory leak right there. Open Task Manager and monitor the RAM usage of the game:) It will probably be around 10GB or something;))
Also, remove any ReShader/SweetFX or other presets and disable the overlays (MSI afterburner, Steam, etc) Maybe it will help.
Sadly this is POOR coding right there so nothing we can do until they patch it out:(
I get the exact same crashes. Sometimes I play 5 minutes other times I play 5 hours:)) Is definitely a game bug though...

Yes, it's definitely related specifically to this game because no others crash.
With the GPU clocked at normal speed it crashed to desktop and gave me the display driver/kernel error. Then I turned shaders to low and it didn't crash on my next play. I'll check the RAM usage out next time.
Petty the coding is so buggy, because they are great games - probably the best in recent memory for me. Ah well, nothing is perfect :)
Well, your 3d fix is pretty close to perfect now - at least that works well :D

UPDATE - Release 0.7
- 26th of April 2016 - Fixed the "SSAO glitch" that was present in some lit areas (Finally ^_^).
- 26th of April 2016 - Fixed the HALOS in Darkmoon Tomb Location.
- 26th of April 2016 - Fixed some lights that I missed.
- 26th of April 2016 - Added F2 key toggle to switch the GOD-Rays ON/OFF - Currently I can't see a way to fix them.
Remember to always "Uninstall.bat" your previous fix before applying the new one:)
Fix is available here: http://helixmod.blogspot.co.uk/2016/04/dark-souls-3-3d-vision-fix.html
Probably a couple of things still need fixing. Didn't get to the end of the game yet and I am trying to fix & improve what I encounter so far;) Doing a lot of research on the side to see all locations + NPC quests + best ending and so on...;) in order to "touch" every area in the game;)
PS: I still get the "INVALID DATA" warning stuff, but Online works perfectly fine. Definitely a BUG on their end... Wouldn't worry to much about it;)

Helifax, just to confirm - you are correct, there is some kind of memory leak. Game plays using around about 3GB memory and then shoots up to just over 8GB when it crashes.
However, for me at least, it only seems to do this when 3d vision is enabled.

Yupp, I see the same thing;) Is not related to 3DMigoto but with the Nvidia 3D Vision drivers in some way...
The 3D Vision driver is highly complicated thing and 3D Vision Automatic more so;) Since I only see it in this game is definitely something triggered by the game when 3D Vision Automatic is used. I think the drivers/game doesn't free some of the resources and then it keeps on allocating more and more memory until you run out and of course it crashes :(
Switching to 2D before loading screens (even when you die) seems to somehow fix it. It gives the game time to free the memory. (Based on a quick test and observation). So maybe you can use this work-around?

Was playing for a while with the Custom Profile and it seemed to be working fine - until it blue screened fighting the second boss on NG+ :D
I have my GPU overclocked, I'll try setting the clock to normal and see if it helps.

However, for me at least, it only seems to do this when 3d vision is enabled.
Thank you:) Really glad it looks awesome;) I think the game looks good in 2D and spectacular in 3D;)
