I had this issue, but I found that turning off the stereoprofile flag in NVinspector fixed it. Unfortunately, I never got my convergence/depth settings right because the sights were still a bit off, and the water was still broken. So I don't know if disabling that flag affected the fix, actually.

Hmm, renaming did only help for the first start, as the driver creates a new profile.
So you should really use the NV inspector, to remove the exe from the profile. It's not really difficult.
And if nothing helps, you should consider a clean driver reinstall or a OS reinstall.
I can't believe, that Fo:NV is so much different from FO3, and that's working good for me on Win10 64bit.

That flag didn't change anything for me. Driver behavior is very strange. Removing the exe from profile really helps. It fixed the Alt+Tab problem for me in FO3.
For the water you can try my FO3 fix, maybe it works for Fo:NV too. You find it on the helixmod blog.
