I don't know if it's just me, but occasionally while running around, the fix just stops working and either goes 2D or runs with the base nvidia 3D handling (in 3D but shaders broken and UI renders at screen depth). This happens to me as well in Watch_Dogs. Requires a full game restart to clear it up completely but sometime it will suddenly return, though usually briefly.

Pretty sure almost everyone experienced this, myself included. Very carefully follow the instructions on the blog about this game too have the best chance of a stable experience.
For me the culprit ended up being the infamous KB patch referenced there, and I was able to play the game even with zoning and had a pretty stable experience in the end. The funny thing is I think recently in some games this patch maybe actually required required to fix similar issues. pretty annoying and wierd.
I believe its also hard-coded into windows 8.1
I also had to force Vsync to adaptive in the nvidia CP otherwise I would get some random elements in 2d

I've put up my latest wrapper on my OneDrive under games folder. Missing FWS currently as I had the same problem with and without FWS.
Running a fresh install of the latest drivers.
My current success rate is rather poor.
Managed to load once without disabling the 3D fix.
It broke when following the whole list of settings on helixmod page.

Since the absolute abortion of Arkham Knight, and witcher 3 still being worked on. I decided to finish up my play through on this game.
To throw a spanner in the works I even installed the "evil windows update" that caused the majority of my issues, since I haven't touched it for a long time.
Lastest updates to the game, Latest driver. Official profile.
Flugans wrapper with the helix fix. (sorry I am actually not to sure at this point which version of the wrapper I am using but its an early one.)
Result, near perfect experience, no issues with rendering or text in one eye. Able to reload and zone multiple times.
The only caveat being I must first start the game in 2d mode and then enable it once I load mysave else I get a Driver crash.
Pretty happy, I must say.

I ended up updating to the very latest update of the 3dmoto version to test and found that I actually had to remove the infamous KB update else started to get incorrect stereo rendering again on startup
But now its back to being flawless again.
I am running my SLI setup activating all displays but only using the primary screen for the game so I wonder if this is a factor that explains it.
I am just glad they actually fixed the issues with the profile. Zoned into multiple maps and cuts scenes in my play tonight and never once had to restart the game =)
