Trouble with Nvidia 3D Vision Surround and 4th Accessory Monitor

Have you tried a program called Lucid Virtu? Its prime purpose is to combine your dedicated GPU and integrated GPU (from CPU) to give you a performance boost in games. Its success is debatable, but a nice side effect is that it lets you plug monitors into your iGPU (which otherwise would have to be disabled in the BIOS).
On my previous build, I used to keep my secondary monitors plugged into the iGPU that way, which was great because they would no longer interfere with games that disliked multimonitor setups. It may work for you too.
Not sure if it's free. I got it bundled with my Asrock motherboard (probably the only thing I ever used - their bundled software is usually beyond awful).

All other configurations will not allow the accessory monitor to be enabled. It will not show up as a monitor you can activate in nvidia control panel.

oh, ok. I suspect if you were on 115x it might have worked. By plugging into the iGPU, it might take it out of nvidia's pipeline so that the nvidia drivers wouldn't know about your 4th monitor, or at least wouldn't care.

Actually, I'm pretty sure it's working because I have the 4th monitor plugged in to a separate card. I just plugged the 4th monitor in to the #1 card, correct position, etc., redid my surround setup. Everything looks normal until I fire up 3D, trine 2.
The 4th monitor works, but the Surround 3D does not. I get what looks like 3d vision (2 images) on the surround monitors, but I get the RED "Attempting to run 3D on non 3D mode,etc." Looking through the glasses just shows the 2 images on the surround monitor. Same thing happens to "Test Stereoscopic 3D" test in the NV Control Panel.
So, for me, plugging the 4th monitor in to a card that is used for surround, stops the 3D on the surround monitors.
EDIT/ADD - Yup, taking the 4th monitor back off of the surround cards and putting it back on it's own card the system works fine again.

3D surround with an accessory monitor is simply bugged in 2 way SLI.

Driver version 332.21 has fixed my problem. I have my 4th monitor running, and 3D vision surround is working perfectly. Not only that, but surround resolutions with bezel correct no longer appear to off set the 4th monitor (which previously would cause things like full-screen flash videos to be slightly cut off).

Nevermind, I was mistaken. While the problem did not manifest for the first half hour or so after installing the new drivers, it now continues to manifest.
It's not a strict switching of the eyes, it's very difficult to describe, but the 3D effect is simply off and very difficult to view.
I thought it might have to do with flash or Chrome running so I tried a number of different things. I tried having the browser entirely on the 4th monitor, both with flash full screen and not full screen and it wouldn't work. I then quit chrome and tried 3D again, still had problems.
The only remedy remains shutting off the 4th monitor. At this point, I'm so exhausted from it, I think I'm just going to disable 3D. It's a shame, but when my options are either never having an accessory monitor - even if I'm not in surround - or not having 3D, it's really hard to give up any way of having an accessory monitor.

I am running tri sli in Win 8.1 and also have the same problem with the accessory monitor. I had thought the problem was specific to Win 8.1 and was ready to install Win 7. I guess I will hold off on that!

We had a big discussion awhile back on Triple SLI, and the conclusion was mixed. Some tests seemed a little better a lot did not. That's what Pirate was referring too.
Now in this test, I'd also like to point out that the part I really care about is [i]minimum[/i] frame rate. Like usual the tester focuses upon average, and really I could give a damn about average. The only thing that actually affects my play is minimum frame rate. You know, when all the bombs go off at once and I'm running. I need it to not lag right then.
If you look carefully at his tables, not the graphs, you will see that Tri-SLI is actually not adding much value in most of his tests for the minimum frame rates. In a couple of tests, the min is actually substantially worse, like in Infinite.
There is some really nice scaling with the third card in a couple of games, particularly Tomb Raider. That demonstrates that there are at least some scenarios where Tri-SLI can improve your minimum frame rate.
So, the upshot is that it seems to be much more variable than I'd expect. In general, it is in fact helping in 3D, but it's not a slam dunk.

The only thing I can figure is that you're using 3 way SLI.
Perhaps the issue is only relevant to 2-way SLI?
