3Dmigoto makes the game crash for me instantly if I force fullscreen :(. I've tried a few profiles and updating to the latest Nvidia drivers. Still nothing. I also tried booting from FFX.exe and the launcher, and using both 1920x1080 and 2560x1440.

You just need to adjust values in ini-file.
In
[KeyCycleView]
key = tilde
convergence = 7.389518, 1.2
separation = 46, 46
type = cycle
try something like
convergence = 7.389518, 1.1
separation = 46, 12
I already did that. At most, I reach the SQUARE ENIX logo. But once it appears, it crashes.
I got a few here showing what I was talking about earlier. Adjusting convergence only seems to change the characters in the small rooms. And then the other two you can see the map stretching outside of the box.
Just noticed your post though. I'll try that out and see if it helps any

Launching the game through steam did the trick xD. I was doing it from the exe files directly before.
Edit: I tried using modified exes to use Japanese voices, which made the game crash. Went back to using the original files, and now crashing again :S.

I found those values and changed them accordingly but now tilde does nothing for some reason. The lines look like this now:
[KeyToggleView]
; key = tilde
; convergence = 1.2
; separation = 46
; type = toggle
[KeyCycleView]
key = tilde
convergence = 7.389518, 1.1
separation = 46, 12type = cycle
Did I mess something up?
EDIT: Messed up the quote, no clue what happened lol
Like I can't change convergence at all anymore. It just seems to be locked. I have "enable advanced in-game settings" enabled and the keys I have convergence tied to do not work. And I think I realized that you wanted me to take lines out of the [KeyCycleView] so that only convergence and separation lines are there? I tried that and it still seems to be locked

Maybe you have some overlays enabled (like, steam or rivatuner or fraps)? They sometimes crash s3d-wrappers.
Or maybe it's some antivirus software?
To ArchSublime:
Well, you can try to use 3D-Hub Player profile. Assassin's Creed IV seems to have some settings like stereo cutoff, maybe they are causing in your 3rd and 4th screenshots.
Or it maybe some fullscreen effect. In that case, you'll need to fix or disable appropriate shader. It's relatively easy to disable specific shader (look for instructions on helixmod).
I'm not really started to play yet, so I didn't encounter any problems myself ^_^

You can use even lower values instead of 1.1 and 12.
Oh, and if you don't see any effect while changing convergence, that might just mean that your separation is too low. That also may mean many other things, but you should try to increase separation first.

I'm actually not using any profiles since I got it working with the files you posted. I'll try it out with the 3D-Hub profile, and restore those lines I took out lol

Oh, I got it. It's just that some scenes are 2D backgrounds with 3D characters. There is no way to render them correctly, so you should temporarily turn off s3d or switch to minimal separation.
To toggle minimal separation, use
[KeyCycleView]
key = tilde
convergence = 28, 0
separation = 46, 0
type = cycle
Still nothing. I've been all morning trying to get it to work. It booted once randomly from the FFX.exe directly (the original English voices exe), but I couldn't reproduce it again.
I have also tried changing the hooking method in 3Dmigoto. FFX-2 also crashes, as well as the credits video. I don't know what else to try. Am I seriously the only one with this problem?

Ar last! I could get it to work consistently by using "full_screen=2" in "d3dx.ini"!!!!!!
I've seen a bad waterfall effect just at after signing autographs in Zanarkand. I'll start fixing that.

And yes, waterfalls and water reflections are broken.
But most annoying is minimap - blue layer is on different depth.
