On first startup I got the black screen too. I think you just have to be patient while the cache does its thing. I think it even took about 5 minutes this time around for me.
I was making the mistake of killing off the black window to soon thinking it had crashed. Maybe go away and get something to eat or something.

I'm actually one of the ones that simply could not get the game to work with the default profile and had to use the BF4 one in SLI. I haven't had this issue since updating to the latest driver though. I think I must have over 30 posts in this thread documenting those frustrations. So I feel your pain.
Vsync settings appear critical, for me I find most stable results with it set forced Adaptive in the nvidia control panel other wise I get some elements rendered in 2d. (Vsync off in game of course).
I've actually been playing this all day today with no issues. This game has been by far the most frustrating for me to get working in 3d vision but its been worth it, some zones are truly beautiful.
Last night I had one issue where the game wouldn't kick 3d in at all. Turning 3dvision off and on again in the panel fixed this.
Yes, reinstalling the driver undoes the changes made to the registry.

Yeah,
Getting this one to work in 3D Vision the first time is a bit of a pain. But the effort is worth it;)
Also it appears to varies from system to system what drivers are best for the game.... which is sad again:(
I had best luck with 353.30/06 on my SLI Surround system. And 350.12 on my 880M laptop.
Setting Vsync to ON/Adaptive/off in Nvpanel and ON/OFF in-game seems to really influences how 3D Vision works with this engine.... I never had SLI issues with the game though....
You need to play a bit and see what works best for you:)

I know this is not a consolation but... I almost smashed my (innocent) HDTV after the latest multiple failed attempts to make my owned games work properly in 3D.
(It looks to me that, the current NVidia driver -353.30- has completely messed these games. I had (and even after reinstalling the 353.06, still have) CTDs on GTAV, W3, DAI...)
Yes, I'm sometimes also extremely tired and frustrated to have to do so many operations in order to make these games work in 3D. (unfortunately, I'm not a software engineer...)
What stops me from loosing any hope (and damaging innocent appliances) is the big respect I have towards the fantastic guys from Helixmod. Their unselfish work keep the option to use our both eyes alive.
It is very sad that NVidia is now working only for cyclopes...

Eons ago, I was one of the happy owner of the first NVidia (almost square) 3D glasses in Central Europe (where I was located at that time). I have become an instant fan of NVidia. Ages have passed and, after a long gaming pause, I was absolutely disconcerted to discover that NVidia no longer supports the needs of people with two eyes. (Compatibility mode is a subversive 3D killer).
Anyway, here are some nice screenshots from DAI to alleviate the sadness (Thanks again to the Helixmod guys!). After two days, I was finally able to start the game in real 3D (for only one map... CTD at the next).

I actually had it break on restart once last night. (some elements in 2d). I eventually got it fixed by toggling Vsync off and playing the game. So although it works in adaptive for me, if it bugs out it seems I have to toggle it to get working.
I also don't clear the cache, unless it breaks. Got best results so far this way.
353.30 is the most stable I have ever had this game run, although I do have a SLI surround setup.
Another couple of tips that help keep it working for me.
* Alt + tab and kill the process when exiting the game.
* If it breaks or you can't get it working. Turn off 3d vision in panel and reboot, then turn back on and set display to "only when 3d programs run."
I have also found in some instances you are best to let the game actually startup in 3d vision if you have having issues with it disabled via registry and it still won't work. Most of the time it loads in fine for me now with it on.
This game is really still a bit of a crapshoot but with these things, I can get it pretty consistent

I fired it up to test my new card, and I'm getting 2d shadows again, hard to tell if its cause of my upgrade or Bioware have broken something.
*edit* Scratch that, ended up being the old Vsync toggle to fix.
On another note I get the same issue in GTA V

The Descent is another DLC and the 1.5GB update was to prepare game for this content.
Anyway the game works like before - you have perfect 3D only if you manage to start it right. Go to Regedit and switch StereoDefaultOn to 0. Then when proper gameplay starts switch 3D on by Ctrl+T.
That always work great for me (The Witcher 3 as well). Remember to switch Shader Cache "off".
Btw: This DLC is less entertaining then previous one but it is still good addition to this great game. I like underground environment with old fortresses. The golden light blasting through walls or sunlight from above makes it look so realistic and that water all over the place dripping on rocks. This engine is amazing looking.

Regarding the new DLC there are a couple of "new" shaders that I noticed and you will defo notice during some cutscenes (decals on some walls). Nothing game breaking though;)
As for the DLC itself... Man the Deep Roads look fabulous in 3D Surround (breath-taking even). As for the content itself... I liked it 80%. Loved the fact that there are new things to do;))
I just kinda wished there were more Darkspawn to fight and the conclusion revolved around them...but well you can't anything you want;)
Still I find the DLC a solid piece of addition to the main game:)
Oh and on WIN10 in SLI you get out of memory DX error in 5 minutes (2x780TI) so I recommend Windows7/8.1

I´m playing on w10 64, with no issues, same as w7, with the 3d fix and flw (running as adm)
It keeps crashing at start with no error. I don't know what i am doing wrong.
-i used DDU and installed 347.09 driver without geforce experience
-i changed things in Manage 3D Settings
-i copied 3D Migoto Wrapper to game folder
-i opened and minimized Flawless Widescreen
I opened DragonAgeInquisition.exe with admin rights, a black box apeared for 5 seconds then it made it fullscreen and it returned me to desktop without any error.
I even deleted my last save in case the save was corrupted. I simply can't get it to work in Windows 10.

Same thing with latest driver ..i start the game, it go's fullscreen, then as soon as the 3d light from the monitor turns on, the game closes without error.
This only happens with the "3D Migoto Wrapper" in the game folder, the game starts fine without the "3D Migoto Wrapper" or using "Flugan Wrapper" but then all the 3d is messed up.

Shader cashe off
disable 3D at load
disable during load screens
alt-tab during load screens.

Try the latest drivers? Working fine for me... if I DISABLE SLI (damn SLI memory leak)...

This is neither complete or the only way to play the game.
It just shows how difficult it is to get the graphical engine to stay in 3D mode.
Thanks to everybody using my assembler it warms my heart.
To have a critical piece of code that everyone can enjoy!
What more can you ask for?
