Just retested the fix here, and it's working just fine. Win7, Driver 368.22, GTX 970. No profile change needed. Nothing different from the current instructions.

I have the Uplay version and apart from one particular problem everything else still seems to be working perfect. Is there anyone here who currently has this one installed who could confirm this problem please? I stopped playing FC4 on my PJ since 720p just doesn't do it justice. (Ive come to similar conclusions with 3 other top notch titles in the past year). But at 1080p im hooked! Another stereoscopic master piece Im very grateful for. Thanks =)
The problem is water reflections and water is quite prominent. So I read DHR determined Max Payne 3 seems to help so I changed accordingly without any difference. Tried the older fix with no luck. Assume the Max Payne 3 profile is suppose to fix this issue?
Is there anything else I could try?
Otherwise if these shaders were fixed manually I would like to hunt them down and compare them to the ones already fixed and see if there are any links I might help with..? After guidance from Masterotako some time back I've finally managed to fix shaders with further help of Helix's algorithms in Dead Space 1 (origin version), by comparison. So I'm finally making some progress. Would like to do the same in ROTTR which also have some broken effects here and there with the official fix that's mostly water related when I go for a 3rd run. I've noticed similar issues in Crysis 3. But Crysis 3 seems to be a bit more complex because it appears to have some form of ray tracing.
These are small annoyances we could live with. But would be nice to have them polished out. I wouldn't mind trying since I currently have spare time and feel a bit more motivated.

Just tested this again, and it seems to be working correctly here. Direct uPlay version, Win10 1803, Driver 419.67, 3Dmigoto 1.2.35 in FC4 fix of 160309. Was forced to get another update, but it did not seem to break anything. IIRC I used 3DFM to setup originally.
Here's what I'm seeing currently:

I seem to vaguely remember that there were sometimes glitches with the game that could be fixed by changed settings and/or alt-tab.
Doesn't seem necessary here, but it will be great if you can pick up some shaderhacking techniques. Just keep in mind that water fixes are some of the hardest ones, so it will require persistence.

Thanks for your reply Bo3b.
I've uprooted the culprit. You could test for yourself and see what I experienced.
Since you suggested Alt+tabbing might help I googled full-screen game launch arguments for Uplay. When I enter the game there are some flashes between window mode and full screen. So i thought it might be related to the problem. Some other guys were also having similar issues which lead me to the GamerProfile.xml found in Documents>My Games>Far Cry 4>blah-blah. So I started playing with the values here which aren't included inside the game.
Under <Custom Quality> you will find "LightingQuality". For some reason mine was set to "low" and I suspect this messed with a whole string of values on which DSS based his fix. LUCKILY my first guess was "high" and not "ultrahigh". Because ultrahigh didn't help when I tested it to make sure. So here I am, Bob's your uncle. haha
Such a simple thing that could have cost me many hours of precious life.
Now I can enjoy it like a boss xD xD xD
Many thanks again to you and DSS.
PS. This might be a good game for VR if you gents are wondering which to tackle. The chopper and gliders in VR would be epic. People would sauce their pants with the random animal attacks. Think it might be a good idea if you guys worked with Ubisoft directly to release something like this. I see só much potential in what you guys are doing and already did.
