Isn't this because you are not actually using the TVs native resolution of 4096x2160?
AFAIK, this is the native resolution on most/all 4K TVs. 3840x2160 is the "recommended" resolution, due to it's 16:9 format.
I don't think that is the reason. It only appears with Nvidia 3D and it dissapears after 1 hour when screen shift is on. But at the same time L/R images are reversed as the whole image is shifted down by 1 row.
It is not present with Tridef or SuperDepth 3D

Probably not the same issue you are experiencing, but mine was fixed by turning off the main monitor so it wouldn't be detected as an extended monitor before launching a game.
