What a extrange issue....Lua file is the same than Xcom fix. If you use the dx9settings.ini from Xcom you will loose the 3D crosshair and some HUD stuff. Try to use my fix, but with the DLL from Xcom file...is the only thing i can think......Also, for other users the fix is working.
Today i will improve the HUD and Crosshair, yesterday i was very tired, so i do a quick look to that and play like 40 minutes (don't found issues).

no success with dll from xcom, neither with lua from xcom but success with dx9settings.ini from Xcom... strange and boring...
I compared your 2dx9settings and will try a mix up...

@chtiblue, try replacing the [General] data from Xcom .ini in to my .ini file.... it's the same (except for one command, that i don't use in my fix), but you can try anyways.

Yesterday i play a little more and found 1 light issue, is render only in one eye (the extrange thing is doing ALT+TAB randomly fix it..LOL)... Too late to start fixing, so today i will look....maybe surface creation mode is the solution.
