Profile for game in my library disappeared with latest driver... what to do?

So I finally installed the latest drivers... no reason other than I was bored I guess. I started up random games to see if they are working in 3D. They are, all except for one, and the problem is that it doesn't start up at all now. It just opens a blank window and that's it.
So I went into "manage 3D settings" in NVCP and discovered that the game shows up in my programs list, but isn't named correctly. Suspecting that something is amiss with the game's profile, I fired up NV Inspector, and lo and behold, there's no longer a profile for the game at all. I decided to create one, and when I pointed the game's .EXE to my new profile, it told me that the .EXE was already associated with three other game profiles (none of which I own). So I deleted the other offending profiles, however my game will still not start.
I'm thinking there must be something else I need to do, like copy a profile from something else compatible with that particular game, but I'm a little fuzzy on how that is done. Or could another possible solution be to rename the .EXE to point to a completely different profile that works? - I'm not sure which approach I should be trying. Can someone point me in the right direction? Thanks.

Surprisingly the 3D was near perfect other than the map. It's sort of a 2.5D side-scroller. I would still like to finish the game someday - hence the reason I'm bringing this up.
Thing is, I don't know which driver version made the profile disappear because I skipped a bunch of driver releases.
I could reinstall the older driver I suppose. I just hate going backwards...

Thank you for pointing me towards that link. Unfortunately I'm having difficulty following since I'm just reading about this at work. I can figure out what driver I WAS on (it was 355-point-something) and maybe that'll get me somewhere.

It might just be a wonky install/GFE issue, I'm running the latest Drivers(358.50) and the Batman: Arkham Orgins Profile is there in NVIDIA Inspector. I exported the Profile [s]but I'd suggest reinstalling Drivers and ditching GFE if it's installed and you're not using it,[/s] afaik not all Settings can be imported via NVIDIA Inspector like they can with GeForce 3D Profile Manager.

Edit: There's absolutely nothing for Blackgate in all the Driver Profile Lists I have dumped ... 320.49, 347.52, 352.86, 353.30 and 358.50.

Okay, I've finally had the chance to tinker with this TsaebehT. I followed the link to your instructions about DarkStarSwords updated XML and applied the changes with the MysteryStereoSetting. Things have changed, but the game will still not launch.
Here's what it did before:
Click on the launch button via Steam - Blank window opens. That's it. The only way to close the window down is to open the task manager and terminate the application.
Here's what it does NOW:
Click on the launch button via Steam - Stereo emitter turns on, blank window opens. Now the mouse cursor changes to the in-game cursor, and the game audio is audible as if it is running normally. But no video whatsoever.
I would suspect a driver issue but absolutely nothing else seems to be running amiss.
I DO have 3 displays hooked up, all of which have stereo capabilities. I disabled all but one just to ensure it wasn't something wonky with the display settings.
I also disabled my antivirus just to be sure there wasn't some interference with it.
One more thing, I also wondered if it had anything to do with my DSR settings (because the blank window that opens doesn't appear to be sized and/or anchored to any particular display), but that still didn't change a thing.

I've seen this happen on a few games.
The solution was to disable logos and intro movies.
Sometimes clicking around randomly on the blank screen, you can click on the menu and proceed to play.
I'll install this and see if I have problems launching on a single display.
Windows 7 driver 358.87 Asus VG278H
When I first launched the game, I got the red text from Nvidia stating that I was trying to run 3D at an unsupported refresh/resolution.
My desktop was set to 120HZ, setting it to 60Hz removed the red text.
There was not a profile associated with Blackgate.
Game launches in 3D without any issues.

3D Vision surround requires 3 identical displays. You can however overcome this with EDID overrides in some instances.
I was going to ask what the game's exe was but after reading what D-Man11 found, it might be running in some odd resolution/refresh combo on your setup. You could try disabling 3D/3D Vision and then change the in-game resolution/refresh, if Ctrl+T doesn't work then disable it completely in the Control Panel before running the game.

Sometimes clicking around randomly on the blank screen, you can click on the menu and proceed to play.
I'll install this and see if I have problems launching on a single display.
When I first launched the game, I got the red text from Nvidia stating that I was trying to run 3D at an unsupported refresh/resolution.
My desktop was set to 120HZ, setting it to 60Hz removed the red text.
There was not a profile associated with Blackgate.
Game launches in 3D without any issues.
EDIT: what does this mean? "I DO have 3 displays hooked up, all of which have stereo capabilities."
3D Vision surround requires 3 identical displays. You can however overcome this with EDID overrides in some instances.

On a few really old games, the movies/intro would bug out using resolutions other than the ones in game. They wouldn't scale and the game would be all white or black, but I could hear the sound. Sometimes alt-tabbing would work to get the options/play screen to load.
The game's exe is call "Game.exe"
One thing that is odd, the blank window always takes up all of one screen and half of the one sitting beside my main monitor. That's what makes me think this is some sort of resolution issue.
(Again, I have tried disabling all but one of my screens to no avail).
